Welcome to Sage Academy, brave parent. You have answered a different kind of call — to stand alongside your child as they become the hero of their own story.
Work through each milestone in order. Download and read every document carefully, answer the comprehension questions to prove your mastery, then sign and upload to Procare. When all eleven milestones are complete, your Compass Badge awaits.
Texas law requires all first-time school enrollees to have current immunization records on file and vision and hearing screenings completed within 120 days of admission. Review the requirements below and upload your documents to Procare before your learner's first day.
What to upload to Procare
Immunization records
Required by Texas DSHS before your learner's first day. Upload your child's current shot records from your pediatrician — or an immunization exemption affidavit if applicable.
Vision screening verification
Required within 120 days of admission. A signed note from your pediatrician or eye doctor qualifies. Most well-child visits include this.
Hearing screening verification
Same requirement as vision — within 120 days of admission. Most pediatricians conduct this at well-child visits.
Copy of birth certificate
Required for enrollment records. Upload a copy to your Procare portal before your learner's first day.

Your provider fills it out and signs it — covering vision screening, hearing screening, and well-child visit in one place.
